Pathophysiology of Post-Acne Erythema (PIE)
Post-Acne Erythema (PIE) consists of flat, pink-to-bright red spots left behind after inflammatory papules or cysts resolve. Unlike brown/black Post-Inflammatory Hyperpigmentation (PIH), PIE is entirely vascular in origin.
During acne inflammation, surrounding micro-capillaries in the superficial papillary dermis dilate and undergo wall damage. Even after bacteria die, these capillaries remain locked in a chronically dilated state.
Capillary Damage Recovery Stages
Track micro-vascular healing across the 3 recovery phases:
Tranexamic 3% + Centella Layering Protocol
Combine VEGF vascular suppression with vessel wall structural repair for maximum red spot clearance:
1. Morning Application: Tranexamic Acid 3%
Apply 3 to 4 drops of 3% Tranexamic Acid serum on clean skin to inhibit plasminogen activator and constrict dilated micro-capillaries.
2. Evening Application: Centella Asiatica (Cica 5%)
Follow with a rich Cica gel moisturizer. Madecassoside stimulates Type III collagen synthesis around fragile dermal capillary walls.
Vascular Recovery Formulations
Dermatologist-evaluated red spot serums available in India:
3% Tranexamic serum to constrict dilated capillaries and resolve red marks.
Centella Asiatica + Ceramide gel to rebuild capillary basement membranes.
